Full Job Description:
General Information:
o Will you accept a first-time traveler? No
o Years of Experience? Minimum 2.5 -3 years of experience (Home health highly preferred; mixture of home health, inpatient/outpatient experience is preferred)
Patient Types:
o Entity county of Yakima, large Spanish speaking community
o Ortho, knees/hip replaces, discharges, deconditions, fall risk patients, neuro patients post stroke
Patient Ratios: Average about 5 patients/day. (25 RVU/week; consists of routine visits and evaluations)
Required Licensure Certifications:
o Gradated of an accredited school of Physical Therapy
o WA State PT License
o BLS
o OASIS certification preferred but not required
Skills required:
o Lift/Carry, Bending, Twisting, general PT skills, mixture of inpatient, outpatient and home health experience.
o Primary Spanish comfortable utilizing an interpreter (in person/video/phone)
o Personal Vehicle and Valid driver s licenses
Support on the Unit:
o Supervision PT; available via phone or computer to answer any questions while onsite
o PTA
o Schedulers
o Insurance auth personnel
Technology/Equipment:
o EMR: Cerner, transition to EPIC in June.
o Dell laptop (windows place) Microsoft outlook/teams
o Apple iPhone provided
Floating: If yes, where?
o Throughout Yakima County
Orientation:
o NEO
o Tiered orientation model check offs before advancing to next tier
Dependent on skill level (ideally, within the first week)
Scheduling:
o Weekend rotation: Sat every 5-6 weeks, 1 day off during the week when working Sat.
o On call? No call
o Holiday Expectations: Yes, Holiday rotation (considered OT)
o RTO: send to NM for approval
o Shift times: M-F flex shift 0800-1430 (4 10s is optional)
o Schedule cycle: Set schedule
Other notes:
o Scrub Color: Any color or business causal (no gym clothes; professional attire only)
o Culture: Small team. Supportive. Looking someone to work independently but also a team player.
Morning huddle; touch point everyday and case conference every Tuesday
